Athletic Policies and Documents

In choosing to participate in athletics, a student agrees to abide by not only the policies listed in the Student Handbook, but also additional policies, including, but not limited to, the policies of his/her coach(es), the policies listed in the Athletic Handbook, and the policies of the Illinois High School Association (IHSA).  (The IHSA policies include, but are not limited to, those provided in the “2021-2022 Key Provisions Regarding IHSA Rules“.)  The Administration will ensure that all additional policies are consistent with the philosophy of Mater Dei.

In addition, in order to practice or participate, a student must have submitted to the office a valid certificate of physical fitness: “State of Illinois Certificate of Child Health Examination” form.  A physical, signed by a licensed physician, physician assistant, or nurse practitioner, is valid for 395 days from the date of the physical examination.

Finally, for each school year, prior to a student’s first practice, he/she must submit an “IHSA Sports Medicine Acknowledgement and Consent Form” electronically via our 8to18 website.
